Note: VA/ TriWest will require a formal referral with an authorization from the VA Medical Center. 

Patient Registration

To expedite your initial consultation, please print, fill out the following New Patient Packet, and bring it to your initial visit.  If printing is not an option, please come 15 minutes prior to the scheduled appointment in order to allow time to complete.
